I consent to the collection, use, storage, and processing of my personal information by UPOU BAMS student Miguel Ferdinand C. Co for the purpose of conducting a research entitled "Promoting Collaborative Reference Sharing Practice through a Web-Based, Database-Driven Content Management System among UPOU-BAMS Students."

My information will be handled in accordance with UP Data Privacy Notice and applicable data-protection laws such as R.A. 10173 (Data Privacy Act of 2012). It will be retained only for as long as necessary for these purposes and may be shared only with authorized UPOU personnel, service providers, or government authorities when required by law.
